Verification of Service.
At the end of an assignment that requires it, complete a Verification of Service (VOS) as required for contracts with the State of Arizona. Not all jobs require a VOS.
Use the link in your confirmation
When an assignment requires verification, your confirmation email includes a VOS link in the Session Notes. Completed forms are emailed to the right address automatically once signed.

Sign on your phone
Download the PDF, then fill it out and sign electronically with an app such as DocuSign, Adobe Acrobat Sign, SignEasy (iPhone, iPad, Android), or GoodNotes (iPhone, iPad).
Print and sign
Print the form, have it signed by an authorized person, and take a clear photo of the signed document.
